Blue8 Group Terms:

AUTOMATIC MONTHLY TUITION PAYMENTS: By registering for a class and signing up for a monthly membership you authorize Blue8 Group Ltd to charge your payment card each month from the date of purchase for your membership level of choice. The membership level you purchase allows you to take a set number of classes each week while only paying a flat fee per month. The number of classes per week is determined by the level of membership you are choosing during checkout. Membership levels are 2, 3, 4 and Unlimited classes per week. CANCELATION: Cancelation requests must be in writing (email works) and must be received at least 15 days before your next scheduled billing date.

CLASS PACKAGES: By purchasing any class package you are pre-purchasing class sessions that may be applied to any children's class offered at Blue8 Dance House that your student is old enough for. The Dance package you purchase will be assigned to you and becomes effective on the date of purchase. The classes in the class pack expire 60 days after the purchase date.  Partially used class packages are non-refundable. 

RELEASE & CONSENT FORM:

In the event of an emergency, I authorize the agents of Blue8 Group, Ltd. to administer or use their discretion in securing medical treatment. I understand that every effort will be made to contact (my spouse), the child’s parent, guardian, or emergency contact if an emergency occurs. I hereby release Blue8 Group, Ltd. and its teachers from all liabilities for injuries or damages arising out of personal injury of any kind.
-Instruction and Contact Consent: Blue8 Group, Ltd. incorporates teaching methods used internationally. There may be times that the teachers may need to touch the child/student to ensure proper body alignment, turn-out, and placement. I give my permission for this instructional contact to be made as part of my training/my child’s training. 
-I hereby grant permission for my name/ my child’s name to be published in the Blue8 Group, Ltd. newsletters/Facebook Page or in local newspapers as a result of participating in dance related activities.
-I hereby grant my permission for my child or self to be photographed or videotaped at any dance related events (parades, performances, class activities, etc.). Photographs may be published in the studio’s newsletters/fb page, in local newspapers, or displayed in the studio. Students will be videotaped during the recital and/or other dance related activities.
-By creating an online account in the Parent Portal and registering for classes, I certify that I am or my minor child is physically able to participate in Blue8 Group, Ltd. classes and do hereby agree that this corporation, board of directors, employers, contractors, assistant teachers are not responsible or liable to me or to my minor child for any personal injury, accident, and/or loss or damage to personal property. I do hereby release this corporation, board of directors, and its employees, contractors, assistant teachers, and agents from any claim or cause of action which may have occurred as a result of my minor child's participation or my participation in classes or as a result of any medical problems known or unknown which may occur in the future. I agree to waive and relinquish all claims I or my minor child may have against Blue8 Group, Ltd.

PRIVACY: We won't share or sell any of your contact information. You may receive emails from us in the future, but if they get to annoying you will always have the ability to Opt Out of receiving further emails.
